Dear Mr. McKenzie,

On or about August 23, 1983, you were administered a flight test and issued a Private Pilot Certificate with a glider rating by an inspector of this office.

The aircraft did not contain conventional glider control systems. Our agency headquarters in Washington D.C. has advised that they did not intend for standard glider ratings to be issued in hang gliders. Also, a demonstration of competency was not conducted regarding an aero or ground tow according to the regulations.

Because of these matters, the agency finds that an error has been made in issuing the certificate. You are requested to surrender the permanent certificate, No. 2345005, and the temporary certificate to this office immediately. Should you fail to surrender such certificate(s) by October 24, 1983, we propose to turn the matter over to our Regional Attorney who will take the necessary legal action to revoke the certificate.

We regret that these unfortunate circumstances occurred, and apologize for any inconvenience or embarrasement you may experience. Please call or come by, if you desire any further discussion of these matters.
